What Causes Wrinkles?

Wrinkles are caused by the loss of collagen beneath the skin surface. Collagen is a basic building block of the skin and provides the foundation that supports your skin's structure. Overall, exposure to ultraviolet (referred to as UVA or UVB) radiation from sunlight accounts for about 90% of the symptoms of premature skin aging, and most of these effects occur by age 20. Collagen is also damaged by other factors, including: the natural aging process, cigarette smoking, and air pollution. When this happens, the skin appears thinner and becomes less elastic, resulting in facial lines, wrinkles, texture changes, and visible signs of aging.

Signs of aging skin
The changes are easy to see: lines, grooves, creases, crow's-feet, bags, pouches, folds and sagging skin. Suddenly you look tired and old, even though you don't feel that way at all.

DermaLift™ Non-Surgical Face Lift

What are AD-SVF injections?

The complex of Adipose (fat cells), Stromal Mesenchymal Stem Cells, and Growth Factors is described in the medical and science literature as the Adipose Derived Stromal Vascular Fraction (AD-SVF).

AD-SVF injections can be an effective way to reduce wrinkles, diminish acne scarring and regain a more youthful appearance. By injecting human AD-SVF harvested from your own body, Dr Dominguez can enhance facial fullness, fill deep creases, soften facial creases and wrinkles, plump lips and build up shallow contours. AD-SVF injections may also fill deep acne scars, cellulite depressions, and breast volume loss.

Unlike other cosmetic injectables that are made from synthetic products, AD-SVF injections do not pose a possibility of an allergic reaction because the AD-SVF tissue is taken from your own body. While AD-SVF injections do not stop the aging process, facial augmentation with human AD-SVF may help delay the need for more invasive procedures, such as a face lift, forehead/eyebrow lift or other types of surgery. AD-SVF injections are also a beneficial addition to other cosmetic surgery procedures.

How are AD-SVF injections performed?

AD-SVF injection procedures are generally performed on an outpatient basis using local anesthesia. The procedure usually takes 30 minutes to an hour, or more, depending on the patient, the volume to be injected, and additional procedures that are being completed at the same time.

The AD-SVF are withdrawn from certain areas of the body (typically the abdomen, flanks, hips, or knees), either by using AD-SVF collection cannulas on a hand-held syringe or through techniques similar to those used during liposuction using special AD-SVF cannulas and suction device settings. The AD-SVF collection process is critical in assuring AD-SVF survial. Traditional liposuction techniques result in the loss of viable AD-SVF.

The AD-SVF are then injected into areas needing augmentation through an AD-SVF injection cannula that is usually passed in and out of the specific areas multiple times. This creates a ‘grid’ of threads of AD-SVF graft.

Dr Dominguez does not centrifuge the AD-SVF since spinning the AD-SVF separates the stroma, stem cells, and growth factors away from the living fat cells and leads to greater AD-SVF graft loss. The stroma, stem cells, and growth factors were once thought of as “imputies” but in reality may be thought of as the netting that holds, feeds, and promotes fat cell survival. The anesthetic fluid and liquid fat is decantered before AD-SVF injection.

Do AD-SVF injections last?

When AD-SVF are properly harvested and injected, a AD-SVF injection can have long-lasting results. However, the “take” or volume maintenance is somewhat unpredictable. Since some fat cells undergo normal degradation, Dr Dominguez may “overcorrect” or inject more AD-SVF than is initially needed, leading to a better end result.

There are many factors that will determine the longevity of the AD-SVF transfer. Smoking and sun exposure will significantly decrease the longevity of the AD-SVF. Rapid or significant weight loss from dieting or illness will also lead to a significant decrease in the longevity of the AD-SVF.

What results can I expect?

You will see immediate results from your AD-SVF injection, and the treated areas may appear too augmented at first due to swelling.

Over time, the swelling will subside, and you will usually be able to see your true results in three to four months. During this period of time, you may lose fat volume from the injected areas as your body metabolizes it.

In general, about half of the injected AD-SVF is absorbed back into the body in three to six months. The remaining fifty percent of the AD-SVF is retained for 36 months or longer. Follow-up with Dr Dominguez is the best way to ensure long-lasting results. A touch-up may be performed after the first three months, if indicated or desired.

How are AD-SVF injections different from cosmetic injectable fillers, like Restylane, Radiess, and Sculptra?

Compared to cosmetic injectables, AD-SVF transfers are a more involved procedure because the AD-SVF is harvested from another part of the body. But, unlike cosmetic injectables, AD-SVF transfers can be used in larger amounts to fill larger defects – and there is no risk of an allergic reaction.

Restylane/Perlane/Juverderm are hyaluronic acid particles of various sizes that add volume by adhering to water molecules. Hyaluronic acid is normally found in joints and as such will be degraded the same as joint fluid. Hyaluronic acid may be used for correction of mild to moderate wrinkles and lip augmentation. Complications of hyaluronic acid injections typically are due to superficial injections resulting in a bluish discoloration of the skin (Tyndall Effect). Clinical effects of wrinkle or volume loss correction typically will last 9 - 12 months.

Radiesse is 30% calcium hydroxyapatite and 70% inactive carrier gel. Normal degradation occurs with clinical effects of deep wrinkle reduction or deep injections for severe volume loss. Clinically visual correction typically lasts about 12 months. Complications noted are large lumps and superficial nodules that tend to disappear within 9 - 12 months. Radiesse is not to be used in lip augmentation.

Sculptra is injectable poly-L-lactic acid. Initially designed for severe fat loss in chronic debilitated individuals, Sculptra is now cleared for use in moderate to severe nasolabial wrinkles. Multiple injection session are required. Complications noted at 12 - 36 months are nodules, granulomas, large lumps, and skin discoloration.

What is Erbium Laser Skin Resurfacing?

An Erbium laser is a high-energy beam of light that can selectively transfer its energy into tissue to treat the skin. In simplest terms, it's another tool Dr. Dominguez can use; a high-tech scalpel, if you will, that allows for great control and finesse. Some skin problems are best treated using an Erbium laser, and some conditions are better treated using another method or technique.

The Erbium laser vaporizes various depths of damaged skin tissue as determined by Dr. Dominguez. The Erbium laser procedure is performed using topical anesthetic solutions and healing is usually two to seven days depending on the depth of laser energy penetration administered by Dr. Dominguez.

Most patients prefer to have the Erbium procedure performed on a Thursday or Friday so they may return to work on Monday. You can expect the treated skin to resemble a sunburn, with sloughing of the treated skin beginning on the second day and lasting up to five days. The underlaying healthy pink skin may be noticeable for up to two weeks, with continued facial skin rejuvenation continuing for up to six months.

Laser devices are used to remove damaged upper layers of skin, allowing a fresh layer to emerge. Two types are often used: the carbon dioxide (CO2) laser and the Erbium:YAG laser. They both work in a slightly different ways and are selected for use for specific situations; for example, CO2 is typically used to destroy large, invasive cancers of the lung, throat, and skin; and Erbium is typically used to resurface uneven, scarred or age damaged skin.

Men, women and teens see Dr. Dominguez for skin revitalization and resurfacing to regain a fresher, more healthy and young-looking complexion. Laser resurfacing is one such resurfacing treatment often favored when skin problems are moderate to severe. This includes wrinkles around the eyes and mouth (crow's feet and lipstick lines), uneven pigmentation, skin blemished with scars from accidents or ACNE.

Dr. Dominguez uses the Sciton Erbium laser as a high tech scapel, allowing for extreme precision in treating acne scars, fine wrinkles, and weathered skin while minimizing damage to the surrounding tissue. The Erbium laser is the preferred technology for treating fine line wrinkles, acne pit scars, and aged skin.

Weekender Peel

The Weekender Peel results in the destruction of up to 30 microns of skin. This peel is intended to eliminate age spots, acne scars, fine wrinkles, blemishes, and poor complexion, while providing laser energy to stimulate new collagen growth for facial rejuvenation. The Weekender Laser Peel is a moderate peel that may require three to five days of down-time for a youthful and lush skin texture result. This peel may be repeated every eight weeks.

What is a Laser Facial Micro-Peel ?
The Laser Facial Micro-Peel is an Erbium laser treatment that was developed specifically for busy individuals who are seeking fast improvement in skin texture and tone. It is a laser resurfacing device that precisely removes the outermost layers of the skin.

What can a Laser Facial Micro-Peel Treat?
The Laser Facial Micro-Peel is an excellent choice for acne scars, wrinkles, pigmented lesions, and sun damaged skin. Some of the most common areas of the body include the face, neck, and hands.

One Laser Facial Micro-Peel at the lowest setting is equal to approximately 6 medical microdermabrasion treatments.

What can I expect from a Laser Facial Micro-Peel ?
You will experience smoother, healthier, more vibrant skin, with a significant, immediate change in skin texture and tightness. Your skin should continue to improve over 6 months of time due to increased collagen stimulation in the deeper layers of your skin; this is known as Facial Rejuvenation.

How long is the healing process?
Healing typically takes 2 to 4 days. The treatment can be done on a Thursday or Friday so you can return to work the following Monday. How many Laser Facial Micro-Peel can be done?
Typically the procedure is repeated 5 to 6 times, spaced 6 to 8 weeks apart for maximum effect. Dr. Dominguez will recommend the optimal number of treatments required for your skin condition. He may also recommend the treatment in combination with other treatments to maximize your result.

What is Laser Wrinkle Reduction?
Non-Invasive 1319 Nd:Yag Laser Wrinkle Reduction is a revolutionary laser treatment that softens fine lines and wrinkles on the face, neck and hands, and improves the appearance of acne scars and stretch marks. There is no recovery period associated with the treatment because the 1319 Nd:Yag laser bypasses the epidermis. Instead, the laser targets the dermal layer beneath the skin's surface where it stimulates collagen production, thickens the dermis and raises depressed areas.

How Does 1319 nm Laser Wrinkle Reduction Work?
During a treatment, 1319 nanometer laser energy is applied in a series of gentle pulses. Without damaging the surface, the laser energy is absorbed by the dermal layer of skin tissue where it triggers a repair mechanism and begins to rebuild and replenish its own natural collagen. Results are achieved from the inside out! Skin will begin to look younger and softer as collagen is replenished beneath the surface.

What is the Treatment Like?
1319 nm Laser Wrinkle Reduction treatments are so quick and easy, you can be treated on your lunch hour. Patients typically experience minimal discomfort and see a blush in the treated area for an hour or so following the treatment. Best of all, there is no downtime. You can apply make-up immediately after the treatment and resume regular activities.

Thermage CPT

Thermage ThermaCool technology was introduced in 2000. Thermage CPT is the third generation device replacing Thermage NXT. Dr Dominguez was one of the first five physicians in the Southern California (Ventura to San Diego) to purchase and receive the newest Thermage CPT. Unlike the Thermage ThermaCool, which required general anesthesia or heavy sedation, or the Thermage NXT which still conducted an uncomfortable heat index, the CPT is safely and comfortably performed in the office by Dr. Dominguez.

Thermage CPT safely heats the deep layers of your skin while cooling the surface of your skin to keep it intact during the procedure. This energy strategically heats the deep collagen fibers resulting in 1. immediate tightening of existing “old loose, weakened, and disorganized” collagen, and 2. the activation of the collagen renewal process resulting in “old” collagen becoming replaced by “new, tight, and organized” collagen over the next few months.

Clinical results are known to vary. Some patients see tighter, younger-appearing skin immediately. Others begin to see improvement after 2-3 months. All continue to improve for a total of six months after the treatment. A few patients with significant laxity and loss of skin elasticity (who do not wish to have a surgical facelift) may need the DermaLift™ Procedure or a second treatment four months later for substantial improvement.

Research reveals that patients treated with Thermage in 2002 and re-evaluated in 2007 have shown consistent results with no deterioration apart from the normal aging process. It is projected that the benefits from Thermage may last as long as 5-10 years.

Which Facial Areas Respond Best to Treatment?

Hyaluronic acid is very popular since it offers the best and most immediate improvement on flat aged lips. For deeper wrinkles and loss of cheek fat, calcium hydroxyapatite works well. Dr Dominguez no longer recommends the use of bovine collage due to the allergic reactions that are well documented in the academic literature. The polyacrylates are also not used due to the development of nodules, lumps, and granulomas that occure 3-5 years after injection.

How Are Treatments Performed?

The injections are an in-office treatment usually taking less than 20 minutes of your time. With the help of a small amount of lidocaine, patients generally experience minimal discomfort. During the treatment a tiny needle is used to inject the filler beneath the wrinkle or depression.

What Can I Expect Following Treatment?

Normal activities can be resumed immediately following treatment. Mild swelling at the treatment site may last a few hours, and you might experience some slight bruising which can generally be covered with make-up if it occurs.

Final results can only be evaluated after any potential swelling has gone down; typically after 48 hours. Dr. Dominguez has trained at the conservative Cleveland Clinic and UCLA, hence he knows that it is always better to err on the side of caution and use a little less filler initially. You can always opt to increase the amount later depending on your results.


Dr. Dominguez began using Botulinum A (Botox) in 1984 to treat patients with surgical conditions such as laryngeal spasms and excessive muscle contractions after Facial Reconstruction.

How does Botox work?

Botox is injected in minute quantities into the facial muscles that cause wrinkles. Within three to seven days the muscles relax and the results are impressive. The effect diminishes after three to four months. Clinical studies show that with routine treatments every 3 months, smaller amounts of Botox may reproduce continued desired results.

Where does Dr. Dominguez inject Botox?

Dr. Dominguez is trained and teaches other physicians to inject cosmetic Botox into the muscles of facial expression. Specifically, Dr. Dominguez is a recognized expert in reducing wrinkles of the forehead, between the brow, crows’ feet, lip edge, chin, and the depressor angular oris (sad clown lips). Dr. Dominguez advises against injecting into the naso-labial region which is known to cause a flat upper lip. Dr. Dominguez does not inject for Migraine or sweat related conditions. What is the treatment like?

Dr. Dominguez uses a small 31 gauge diabetic type needle. There may be a slight sting and maybe a mild bruise at the injection site, but this is uncommon. Bruising appears to be more common in patients taking aspirin, ibuprofen, or blood thinners.

Are Botox treatments safe?

Botox is safe in skilled surgical hands, like Dr. Dominguez. Malpractice literature reviews show that untrained practitioners (Family Practice, Pediatrics, Ob-Gyn, RN’s) have the highest rate of adverse events (eye muscle or eyelid paralysis). Dr. Dominguez teaches other physicians to only inject the minimum amount necessary to produce a softening of the muscles, thus resulting in a younger, smooth and natural appearing face. Dr. Dominguez says “instead of injecting more volume of Botox and risking an adverse event, physicians should instruct their patients to return after 2 weeks for a “touch-up” if more Botox is desired.” Botox should not be injected in pregnant women, or those with known neurological diseases.

What are spider veins?

Spider veins are small, thin capillaries that lie close to the surface of the skin. They are connected to a larger venous system, though they are not an essential part of it since they do not actually carry blood to the heart.

Spider veins form when excess blood backs up in the blood stream. The excess blood creates pressure. The body releases the pressure by creating new veins on the skin's surface. The primary problem with spider veins is their unsightly appearance, but they can also cause aching, swelling, burning and night cramps in legs.

Millions of men and women are bothered by spider veins-those small, unsightly clusters of red, blue, or purple veins that usually appear on the thighs, calves and ankles and occasionally on the face. An estimated fifty percent of the adult female population and fifteen to twenty percent of men share this cosmetic problem.Many factors can contribute to the development of spider veins, including: pregnancy, weight gain, chronic sun exposure, activities that demand prolonged standing or sitting, heredity and certain medications, such as birth control pills or hormone replacement medication.

Laser ablation requires no anesthetic; pain is minimal and is described as “a hot rubber band snapping against the skin.” The procedure takes fifteen minutes to an hour or more, depending on the number and size of the veins being treated.

What should I expect after treatment?

Following laser treatment, the treated area is wrapped in a compressed bandage or stocking, and the patient is sent home. There may be slight itching for a day or two after the treatment owing to a mild release of histamine from the treated blood vessels.

Chemical Peels

What is a Chemical Peel?

Chemical Peels have been used for decades to revitalize and resurface skin. They involve applying a chemical solution to remove the damaged outer layers of skin and reveal a younger, clearer, more radiant complexion. They are commonly performed on the face, chest and hands.

Light, medium and deep peels penetrate to different levels and, consequently, produce different aesthetic results. Generally, the deeper the peel penetrates, the more profound the results, but the more lengthy the recovery period.

We offer a full line of light chemical peels that involve the use of either buffered Trichloraecetic (TCA), Alpha or Beta Hydroxy Acids (AHA/BHA). They are free of color additives, fragrances, harsh alcohols, comedogenic oils and other ingredients that are known sensitizers.

Chemical Peels May Improve:

Optimal improvement in skin texture and color is obtained with multiple peels performed two to four weeks apart. However, a single peel performed a week before a special event will give your complexion a healthy, radiant glow.

How Does It Work?
The process usually takes 30 to 45 minutes and although some patients' faces may seem a bit pink afterwards, there is no "downtime" and makeup can be applied immediately after the treatment. You can return to your normal schedule right away (though we recommend allowing the skin to breath for up to 24 hours post treatment). Two to three days following an ultra light peel, patients may experience temporary flaking, dryness and scaling, which can be controlled with a topical emollient.

Silk Peels

“SilkPeel” is a breakthrough treatment that combines a diamond tip micro-dermatome with deep delivery of hyaluronic(hydration), or salicyclic(acne) solutions to improve and revitalize your skin. SilkPeel leaves your skin looking better and feeling fresher. This treatment is great for ACNE and “tired” skin.

What is a SilkPeel treatment?
A SilkPeel treatment requires no anesthesia. Using a precision hand piece, the gently abrasive action exfoliates the outermost layers of the skin. Dr. Dominguez can control the power of the diamond tip and flow of medications from a gentle buffing of the skin to deeper exfoliation where needed. At a low power, a SilkPeel treatment is comparable to results similar to those of glycolic acid peels. A full-face treatment generally takes 10 to 15 minutes.

What results can you expect?
Following the SilkPeel treatment, your skin appears lightly pink, but there is no oozing or obvious redness as with traditional dermabrasion treatments. Some patients say their face feels as if their skin has been mildly sun- or wind-burned. The skin may peel. In the case of a deeper SilkPeel treatment level for more blemished or thicker skins, the skin will appear slightly red and swollen from a few hours to a few days. Similar to a light peel, the SilkPeel procedure can improve fine lines and wrinkles, blackheads and whiteheads, superficial pigmentation, and give the skin a healthy-looking glow. A SilkPeel treatment may be used in conjunction with chemical peel or laser skin resurfacing.
